I am able to bill the following insurances for routine eye examinations and contacts if covered: Please verify with your insurance that you have this benefit available. Some health insurers subcontract routine eye exams out to secondary vision insurance companies that I cannot bill such as VSP .(They don't always explain this)

Aetna
Alliant Group Health Plan
Carpenters Trust
Davis Vision
Eyemed  (Exam only)
FEP Blue Vision (Federal Employees)
First Choice (Most but not all plans)
HMA
Lifewise
Northwest Benefit Network (NBN)
Painter's Health Trust
Premera Blue Cross
Principal
Puget Sound Electrical Workers Health Trust
Regence Blue Shield
Spectera/ United Health Care Vision
Superior Vision
Sterling Medicare Advantage
Uniform Medical Plan
United Health Care Medical

Note: I am not a Vision Service Plan (VSP) provider. Unfortunately they will only allow providers that sell glasses themselves to be in their network. Also, they will no longer pay directly to out of network providers and some VSP plans will even not reimburse at all if you go to an out of network provider.  Please contact them to find out if you can be reimbursed for seeing an out of network provider.  With VSP I must collect full payment at the time of the exam. We will gladly bill VSP for you to get reimbursement if it is allowed by your plan. They usually reimburse about $45.00 for an exam.

Medicare does not pay for routine eye examinations. They do pay for medical eye visits at 80% after your yearly deductable has been met.

If your insurance provider is not listed on this page full payment is due at the time of service. I accept Visa, Master Card, Discover, American Express, Checks, or Cash.

I can give a reimbursement form to you for some insurance that I cannot bill directly. Check with your insurance to find out what they will reimburse if you see an out of network provider.

Thanks
