I am able to bill the following insurances for routine eye examinations and contacts if covered: Please verify with your
insurance that you have this benefit available. Many health insurers subcontract routine eye exams out to secondary vision
insurance companies such as Eyemed, Davis Vision, or VSP. (They don't always explain this) I also bill medical insurance
for medical eye problems. Some medical insurance requires a referral from your primary care doctor before you can see me.

Note: I am not a Vision Service Plan (VSP) provider. Unfortunately Vision Service
Plan will only allow providers that sell glasses to be in their network. Please contact VSP to find out if
you can be reimbursed for seeing an out of network provider. With VSP I must collect full payment at the time of
the exam. I will gladly bill VSP for you to get reimbursement if it is allowed by your plan. They usually reimburse for
about half of my exam fee.
Medicare does not pay for routine eye examinations or refractions. They do
pay for medical eye visits at 80% after your yearly deductable has been met.
